Though they weren't THIS bag. Biggest was 515 pounds at 16 years old.

I don't know how they do it, but the ones that can't move still manage to get shit underneath them. Cheerios, cheezits, other dry foods that can sit for god knows how long without rotting away. They can't fucking move let alone roll over too much but they STILL get food underneath them. And it sticks to them. It gets all over our tarps that we use to transfer them. It's fucking disgusting.

They got a call for general weakness. When they arrived they found a very large man sitting on his couch. The smell was pretty bad. Feces, urine, body odor, and a pile of cigarette butts and ashes next to him. Plates with rotten food. Two weeks ago he became too weak to get off the couch. His roommate had been bringing him food and drink. Mostly junk food and beer. He told them he was too embarrassed to call earlier but realized that if he didn't get help he was going to die. The crew called for a lift assist because they couldn't move him by themselves (only two of us on a unit and it was two girls at that). We didn't have an available unit to help them so our dispatchers requested assistance from county who sent a fire engine with a 4 man crew. 3 men basically because the captain sure as shit wasn't going to help. That's what his crew is for. Well they arrived making the usual jokes about us needing help, but when they got inside the mood changed. They couldn't handle the smell so they put their air packs and respirators on along with their bunker gear to keep any nasty shit off of their clothing. Our crew doesn't have that stuff so they suffered through it. Only problem was that he was even heavier than they thought. After two weeks of sitting in his own shit and piss the couch had begun to degrade below him, hiding his true mass. Well, firemen are buff mother fuckers so they hoisted him up and over to the stretcher. Removing him removed the plug that contained the true stench. When it hit one of our crew members she actually threw up in her mouth and ran outside to spit it out. But, they got him onto the stretcher and into their ambulance. It was then that our crew noticed the maggots on his feet. They were eating his dying skin. They took him to the hospital with the windows rolled down and the exhaust fan running but that fucking thing is useless for small smells let alone one like that. 1/2

When the got to the ER the nurses directed them to the decontamination room. With the help of 4 more nurses they moved him over to a bed where they began the process of cleaning him before they would allow him into the main hospital. The nurses weren't exactly kind about it. Usually they'll fake being friendly and ask what brought the patient to the hospital etc. but this time they weren't having it. They blatantly asked why he let himself get like this and why he didn't call sooner before he "smelled so fucking bad".
At this point our crew returned to the ambulance and spent more than an hour cleaning it. Our supervisors are cunts and usually wont let us go back into HQ to clean our trucks (i spent half an hour cleaning vomit out of my truck consisting of Patron and shrimp cocktails with a broom and a spray bottle). They couldn't get the smell out of the tarp so it had to be thrown out. (It's actually a "soft-stretcher", a tarp with handles and reinforcing material)
Finally the crew had enough. It was a training crew. One was a field training officer and the other a new hire. The FTO called our supervisor directly and convinced him to bring them in. They spent an hour bleaching the inside of the truck. It was taken out of service for a day to let the smell dissipate.

It was the new hire's first day in EMS.
I have a few of my own stories but none quite that gross.

I work in aged care in my country and these people wind up in our facilities. I really struggled with this question for a long, long time. The best answer I've come up with is that they're children.
No impulse control, no motivation, they lash out and have very little sense of shame or accountability. They also tend to be the rudest creatures you'll ever meet. I've had to deal with 6 such people in my particular facility and only one women had any sense of how much of a burden she was and would occasionally apologise.

I picked up a guy from the hospital ER one time to take him home. He had dementia, had no idea what he was doing. When we got there we discovered he had pissed himself in bed. Not too unusual, but somehow this man's piss defied logic and soaked all the way up his back and into the pillow that his head was on, soaking his hair in piss despite the fact that the bed's head was raised. No way it could soak that far up. Getting him home was a clusterfuck but that's not the real story.
About 3 months later we get a call for a fall. As we roll up to the scene I see a man in a wheelchair with firemen around him, must be the patient. But he looks familiar. My partner gets out first and makes contact then I get out and start to prepare the stretcher. I can smell urine. It's extremely strong. Then it clicks. He's the guy who pissed himself in the hospital. No big deal though, it's just pee. Well i'm preparing the stretcher and a fireman walks over and starts apologizing saying how they tried there best to clean him up. What. I grab a sheet and gloves, keep getting ready and my partner walks over and asks me how many blankets we have. Three. She tells me to grab them all and lay them on the stretcher. Turns out the urine smell wasn't him. He's covered head to toe in his own shit. Shit all over his hands and face, chest, legs, everywhere except his hair. He has white hair. He had gym shorts with little holes that acted like a colander. The smell is horrendous. They had taken off his shirt which cleaned him up some, but he was still covered. But, we loaded him up and took him back to the same hospital I took him out of a few months ago. When we get there it turns out they're extremely busy. We sat and waited for literally an hour to get him into a room. Down the hall there was another patient who had been arrested and was accompanied by multiple police officers. Every few minutes one of the cops would poke his head out with a disgusted look on his face.

The crew got a call at the local prison though I don't remember what the dispatched complaint was. When they got there they learned that an inmate had his heart medication changed and it resulted in him having a MASSIVE erection which lasted far too long. It was becoming painful (I believe it's because the blood in the penis doesn't circulate and after time the oxygen is used up and the tissue starts to die). He was accompanied by three officers, one male and two female. Gender didn't matter though because nobody could stop staring at this man's giant dick. He had to lay sideways on the stretcher or else the pain was too great. They got him to the hospital and as they wheeled him by this man's dick was turning heads. His bright orange jumpsuit was drawing attention to begin with, then they saw his engorged member. They got him into bed and one of the ER docs walked in and started discussing what needed to be done. Basically they have to stick a massive needle into the already painful penis and basically drain the blood. The kicker here is that he's a prisoner. He's not allowed to receive pain medications. The only male officer with him stepped up and allowed them to give him a small amount of medication though.
An hour later they dropped off another patient in the ER and the prisoner had been taken into the trauma bay for the procedure but they could hear him screaming bloody murder the whole time.
