The justification given for this is that he was in a car accident. So the doctor, thinking Dale IS Dougie, would already be familiar with the fact that "Dougie" spaces out like that.

There are four reasons I think are plausible for why Dale is in this fugue state:
1. Being trapped in the Lodge for 25 years has just completely mindfucked him.
2. Traveling between the purple zone and the real world put him into some kind of state of shock
3. As long as EvilCooper is alive he can't "wake up"
4. His body and consciousness are separated and he won't "wake up" until October 2

It's not completely clear if Doppelcoop is Bob. The only thing that suggests that he is is the finale of season 2 when he looks in the mirror. What it seems like is that Doppelcoop is his own entity that is basically anti-Cooper. Cooper wanted a simple life and to help people so Doppelcoop wants an extravagant lifestyle and to hurt people. Seems like Bob only uses him as a vessel of some sort but he's not controlling him like he controlled Leland.
