Did the writers just forget the whole rules governing what happened when she first ran into the faceless men? A ritualistic cult/service that doesn't kill without orders and sees that a sort of status quo needs to be kept. Season 4 was the last season that was generally faithful to the books. They changed things in the show for sure in early seasons, but mostly inconsequential. Example - Season 1/book1 Ned and Jamie have that mini duel when Jamie goes to capture Ned before he gets beheaded, in the book they don't actually fight but end result is the same.

After season 4 they started running out of book content and doing more of their trash OC. Last good scene in the show was probably Tyrion's "Fuck you" speech to King's Landing
