You have two choices: male and female brains actually differ intrinsically, on a biological level, so some people will require a sex-change, or male and female brains only differ because they are usually housed in bodies that are raised different, and then a sex-change is purely cosmetic, a more extreme version of changing your hair color or working out.

What really bothers me about the whole transgender thing is that LITERALLY NO ONE could ever define what being a woman means as a gender. I mean since they separate gender and sex, they can't say it's about the vagina and the DNA. So what does that leave? They can't define what a woman is, what makes someone feminine, why exactly they need it. Which shows the whole gender thing is just a social convention they should be fighting against. I've asked a lot of trans people as a survey and no one had a real, convincing answer of what being a woman means. They all say bullshit like "you just know" or "it's a feeling" or "it's when you are feminine" or "it's when I identify as a woman".
If they were just about dressing like the opposite sex or about getting rid of gender norms, it would make sense. If they were open about it being a purely aesthetic thing it would make sense. All they do is just become parodies of women, trying to be as much of a stereotype as they can.

It's hard to fit trans people into stories. And their lives are probably very unique so it will only be relatable to very few trans people. And the rest of the public simply isn't interested in this stuff. If they are curious about trans people they will rather just watch a documentary.
