How can nepotism pay off if you don't pay for anything and have it hand it to you for free?

The only time it ever has. This isn't to say that there haven't been other really talented second or third-generation filmmakers who got their foot in the door thanks to nepotism, but the only time someone's career was demonstrably *fast-tracked* by family connections *and* he proved to actually be talented was pic related.
