The movie didn't address it but the book actually addresses this point and why no one cared that Ratchet was an uncaring bitch.

Basically Ratchet ran a tight ship with her dictator style way of running things. As cruel and horrible as she was, the people stuck in the ward were properly cowed into submission, which meant that they didn't cause trouble. Which for Ratchet's boss, meant she was doing great and was given free reign to be a cunt to everyone in her care "for the greater good".

The book also explains what happened to her after Jack Nicholson's character choked her out (which the movie skips over). He damages her voice box so that she can no longer talk any louder that a whisper. Which killed her power to kill the inmates in line, since her bitchy voice was instrumental in allowing her to bully and browbeat patients into submission. She ultimately gets demoted once she goes back to work, as the patients full on refuse to obey her and without her bitchy voice, she can't bully them into submission
