The last day was literally one day of hundreds of years of dark shit. In the audios he straight up commits genocide, murder, and even kills other Time Lords.
Even if that's not canon, he renounced the name before he regenerated into War, changing the last day has nothing to do with changing who he was and what he had done in the hundreds of years prior. He only claimed to be the Doctor again in that moment.. Not changing before or after. He did something the Doctor would do, but he's still not the Doctor.
