It makes perfect sense to me. Who makes up a large portion of the audience for Scrubs, or indeed any sitcom like this? Women. Middle/lower class women, many of whom are nurses (an overwhelmingly female profession, of course) or are friends with nurses. None of them are friends with doctors.The writers are just playing to their audience.

Actually, they joke a lot in the commentaries about Neil Flynn going off script so that after a few seasons, they'd just write "Whatever Neil says" as his lines on the script.

It would have been better if they stuck to the original idea that the Janitor wasn't a real person
