I think everyone wants the appearance of what were naturally attracted to: strong jaw, full lips, high cheekbones, etc. Maybe when the surgeon is discussing it with them, they have such a distorted or unrealistic expectation that they choose extremes. Or maybe they just want to feel like they've gotten their money's worth. Whatever it is, there seems to be some delusion behind the people who take it this far.
