Basically it's Butch earning the watch by going thru a microcosm of the horror and suffering his father went to. Marcellus getting anally raped is a clear Christopher Walken's character having to shove the watch up his ass to protect Butch's family legacy.

At the end of the segment Marcellus lets Butch go despite Butch fucking him over and causing all this shit in the first place, because Butch saved his life they were for a brief moment like POWs.
