Family medicine? I never study for those and I always manage at least 80% grade. Considering family medicine is supposed to be based on the "20% of the work gets you 80% of the results" rule, it's a pretty good estimate of how good a doctor I'll end up being.

Chasing after points is just pointless. Diminishing returns. I'd rather sleep and enjoy my free time. C = MD.

Medical education should be designed around that simple fact. We should be having easy, no-pressure anatomy classes every single semester. Instead, they teach all 200+ bones, 600+ muscles, ligaments, blood vessels, neuroanatomy, all in one year, and then you're supposed to know everything forever. Well, I've forgotten nearly all the useless stuff I learned in anatomy but never actually used in practice.

What's the point of this bulimic learning, where you stuff your head full of facts, trigger your brain's reflex and vomit it all on the exam, only to forget about everything afterwards? It's pointless. I refuse to partake in it.

My OB/GYN rotation sucked. Well, they all sucked, but that’s besides the point. Anyway, I got to stick my fingers into nasty coochies that were infested with STDs such as HIV, HSV, and countless others I don’t really care to mention at this point.

You see, the patients that I saw really didn’t give a shit. That is, they didn’t care about their hygiene and I honestly wondered if they had showered within the last 3 days. They were homeless, abused IV drugs, intoxicated by 9 am each day, and had sex for crack. They would also show up for their first prenatal care visit at 36 weeks gestation…lovely. “I want da best for ma baby” they would say. “I betta get da best care for ma baby”.

Do you have any vaginal discharge? Loss of fluid? Contractions? Is the baby moving? These were questions I had to ask, but I really didn’t care about this stuff. I had to ask this crap because that’s what they told me to do. This was the right way to do things. Be honest – would you really care to know about some nasty-ass discharge that some crack whore had for the last three weeks? I didn’t think so.

True story: I was in OB/GYN clinic on the first day of my rotation. This crack whore shows up who’s 37+ weeks pregnant with no prenatal care at all, and it turns out that she’s HIV and HepC + (wow, I’m shocked). There’s no telling what else she’s got…anyway, I’m supposed to go in and work her up, do the pelvic exam, etc. Upon questioning her and finding out that she’s:

1). intoxicated at approximately 9am
2). Hep C+
3). HIV+
4). Has a boyfriend in the exam room who’s also intoxicated at 9am

so I walk out of the exam room and tell the resident “I’m not doing the pelvic exam on this chick…sorry man, I just can’t do it.”

He says that’s cool and all, and I go on about my business. God, I can’t imagine people who actually want to go into OB/GYN.

He gets to work at around 4:30 am every morning and usually doesn’t get to leave the shithole they call a hospital until 8 or 9 pm. Yeah, those hours aren’t made up. Those guys must eat, sleep and shit in the hospital since they’re never home. It’s no wonder that surgeons are so bitter, since the next-door neighbor who has regular hours is home banging that surgeon’s wife every evening.

My med-student friend goes on to tell me that sometimes he finishes his work early in the day – around 2 or 3 pm – but cannot go home or go study despite the fact that there is no clinic and no cases scheduled in the OR for the rest of the day. Absolutely nothing going on, and he has to sit there interacting with the miserable residents until 8 pm or so when Attending Surgeon Dr. No Life finally finds time to round on his inpatients. What a huge crock of bullshit, but the unfortunate thing is that this is very common in medical training. I would say that approximately 50% of the time you spend in medical school and residency is time down the drain. Wasted minutes…and we’re paying for it.

He even decided to bring this up with the chief resident on his service. As expected, they were completely non-responsive to his concerns, only wanting to cover their own asses. The reason given for the students having to sit there an additional 5, 6, or even 7 hours was that “the attending will get mad if he asks where the students are and we’ve sent you home. We’re just keeping you here so we don’t get in trouble.” Well, Mr. I-Like-To-Kiss-Attending-Ass resident, that’s a piss-poor answer for keeping students on the service. I guess you’ll be sending them down for your candy bar and soft drink.

There you have it folks, a day in the life of a 3rd-year medical student on the surgery service. In between getting yelled at, physically hit on the hand for holding a retractor incorrectly, and pimped until his ears bleed he gets to fetch the resident’s lunch and dinner. Money well spent.

I have no use for the knowledge of delivering a child. If a woman goes into labor close me I’ll just do what every other normal person would do and call 911. It’s pretty simple.

After watching a dozen or so vaginal deliveries, I have to say that they are far from beautiful. Cats, dogs, horses, cows and all other mammals have live births. Why is that a human birth is so “special?”
