I've done both.

As a "waiter" I have to spend the entire stay with you, make sure you're happy, get your food correct, ask if you need anything else, check on you periodically, and give the best damn service I can to keep you happy.
As a cashier you press a few buttons, do the transaction, and someone else gets you the food, and it's up to the managers to keep you happy

This, I would like to add that if the service is exceptionally bad I'll leave a 25¢ tip. More effective in letting the waitress know that you felt their service was shit as opposed to not tipping at all.
