I find in most restaurants you have to ask for 1 level of cookedness up from what you actually want. medium rares normally come out rarer than I would like

I'll bite the bait. I'm not one of those militant steak purists, but I'll say that if you'll only eat steak well done, steak is not for you. My first medium steak after growing up with nothing but well-done was like reaching Shangri-la. I still have an aversion to anything rarer because of my upbringing, but even medium is a vast improvement.
