It shows the Doctor's family (which Moffat had already done in Listen, of course), opening the very can of worms you claim he'd never open. I suppose the Gods, Great Old Ones, stuff like Zagreus, the Divergent and Guardians were the 'there's always a bigger fish' because the TLs are already technically able enough to deal with most everything so they needed a bigger threat that couldn't be solved in ten seconds flat.
I guess Rassilon's oversights like Daleks etc rising to such a threat in the first place even with the apparent foresight offered by TARDISes could be justified in universe but it might be hard if they don't have time technology to compensate (I mean most EU stuff just gave a whole bunch of races time technology so they offered a 'threat').

But they already had a better solution to that: the Doctor doesn't just call on the Time Lords to fix things whenever he finds a problem, for lots of very good reasons—they're generally anti-interventionist; they're sometimes corrupt and on the wrong side; they're so stagnant that they can't use their immense power effectively (when someone like Omega shows up, the way they deal with it is by calling him…), the last time he tried that they put him on trial and forced him to regenerate…
And of course RTD (and the EDA novels before him) gave us an even better reason the Doctor can't just call in the Time Lord cavalry.
So on the rare occasions when they did a "biggest threat ever in the history of everything" bigger fish story, it was unnecessary, and usually fell flat. And the same was true for RTD trying to up the scale for each year's finale.
Even in the comics, where they cranked up the scale escalation to comic-book levels (with being even more powerful than the Guardians), most of the stories didn't focus on those godlike beings; the Doctor was usually just facing Doctor-scale problems every month.
