Prout is an alien that needs a host he took Kevin Spacey's and that is why he says he can only take one person back to K-Pax. He exchanges bodies and takes Bess or web her name is and leaves Kevin Spacey's knowing he will be taken care of by The Dude.

Another piece of evidence of how he is a body snatcher is the bird scene, Bess says that he is the bird, and she is right, Prout took the bird's body to give the sick guy hope and basically tricking him into healing himself.

For the people that say he isn't an alien at all, how dense are you? It is clearly evidenced in the scene where he draws the map to K-Pax, no human but top scientists even knew those places existed.
