Bodyguards usually keep their distance but at least have someone to keep an eye and make sure no manlet shoots at you while shitting.

Tyrion broke into a secret passageway to his chambers that Tywin himself made for transferring whores to him (no joke). This passageway lead to inside the chambers rather than the corridor outside or whatever, so no guards would notice.

You might remember from a scene in season 1 of Jaime being stood outside king Roberts chambers. That's basically what all guards did, not actually be inside.

Tyrion found the passageway because of varys. Later on when cersei discovers tywins body, she has the whole tower burnt down to hide the fact that he had a passageway for whoring
