The question of whether the ending is reality becomes unclear when you take into account that Jacob is learning about the government's experiments within his own hallucination. Nothing adds up and I think the ambiguity is intentional.

Though IMO, Louie's description of heaven and hell is a thematic unpacking of everything this movie is really about. I love that scene. Louie is a beautiful character.
