Real story: Alfred was pursuing a bandit in Colonial Burma. He failed in his task, and was forced to watch malaria, dengue, diphtheria, cholera, and other nasty diseases from the region slowly kill some of the men under his command. Some of these undoubtedly were men who survived the brutal engagements with the Japanese in the Burmese front. He contracted some of these illnesses himself and went insane. He started seeing rubies everywhere, and so he assumed that the bandit was a madman, when in fact he really was a local who was smart enough to hide his wealth until Alfred and his unit were too sick to pursue him anymore.

Bruce Wayne pursued the Joker by following advice from his hallucinating butler with PTSD.
