It's split into two kinds of human evolution if I recall.
One part is about the humans who stay behind and how they become more animal-like as the millennia drag on.
The other part focusses on those who went into space and their increasing transhumanism to the point that they're unrecognizable.

I really hated Alien Planet. They tried to set it up like a film, going "ooh, what are we gonna find?" when it should have been set up like a nature documentary and explored the plausible possibilities of alien life.

The Future Is Wild was great.
