The area around Madrid has some very nice medieval cities (Toledo, Avila, Segovia) and some other stuff (the Franco monument, El Escorial). I don't know anything about the northwest of Spain though.

Then I think you have chosen the right places, make sure to visit Segovia, Burgos, Salamanca and Toledo.
About small villages, I recommend you to go there on summer or only when they celebrate something or it gets pretty boring and repetitive.

There are more than 4 languages in Spain. They just don't have literature and their population sink so they were naturally replacrd by Spanish
