I really can't get over how good this one is. Every time I sit down to really read this or The Village I'm overwhelmed by the atmosphere they create. The biggest difference between the two, in my opinion, is that the drawbacks in The Village are all really cool and I'd take every one, whereas with Urban Unease the hostile phenomena are legit terrifying for the most part. DSA is/was a gifted motherfucker. Never thought I'd be imagining my life in fucking purgatory, but here we are.

My biggest suggestion is to rearrange the content and make the headings more clear. Try this....
Page 1- Larval Stage, holy shit this thing was in me. Where? What does it mean for me? Am I going to die? I think which Organ should go here, then add a separate power section on Page 3 for which organ you selected
Page 2- Appearance and Relationship, how does my parasite see me? A tool? A parent? A partner? Classic parasitology etymology works here, commensalism, mutualism, parasitism. Make it like sub, equals, dom.
Page 3- Powers, duh. Both generic and organ specific
Page 4- the future, possible implications of your shared mortality

Upon a second quick scan, I'm a bit confused. It reads as if the creature has a physical body, but remains an "inhabitant" somehow. Is a piece of that creature still within you? Or did all these changes take place during the larval stage? If it's the latter, the power section needs to be written in past tense. Additionally, referring to them as "former inhabitant" would facilitate understanding of the situation

Spinal rooting is the best option. Nearly quarter of the upgrades are locked to the option, and they're all pretty good too. Hell, you only have a single tier three option if you don't pick it. I mean, it makes sense that the telepathy based things require the telepathy root, but it ruins balance. Especially because only the heart root unlocks anything, and that's one thing to spinal's three.

for me a lot of options with autism stats tend to stress me out and I may even drop it or ignore all the stats, or go full autism mode and trying to make the most powerful build even if it's not something I like.
Because, if there are such powers surely somebody else has them, and I don't want them to kill me, or to conquer my village while I'm a helpless craftsman.
I prefer CYOAs where there's less focus on spending and calculating points and mor on making choices that can feel organically connected and build up to your story.
Also, if you're making one where there is the options to acquire destructive powers, make sure that there is some kind of protective choice that other builds can make so you don't have to be completely helpless if you decide to make something that isn't a murderhobo.
