It does make sense, actually, it was just drawn horribly. As for the activation method, well, you've got me there. I would assume either magically activated or have something that is triggered by potentially heavy breathing from adventurers while pearing through the door in between encounters. *shrugs*

It looks like the forehead bumps a loose stone or brick when you put your eye right up to the hole (or whatever, it could just be a small button) which hits the blade or ball bearing, which swings the blade itself. You'd need quite a bit more space behind the wall, and most likely a very curved blade (hook would be best, I think), but it would work.
