Rovagug got a little too restless and started breaking out, so the gods activated a fail safe which involved ripping open time and chucking the entire planet into a pocket dimension beyond even their reach. This had the unfortunate side effect of causing the Gap, but considering the alternative the Gods are kind of OK with the trade off. The demons and efreets play along because the Gods have made it clear they're more than happy to put their stomping boots on if anyone tells the mortals what went on

Well, it started as a decently-interesting kitchen-sink rpg setting. There wasn't necessarily a lot of thought about the implications of all these things rubbing up against each other in the same world, but you could tell that the writers put in things that they liked, so you got everything from 30s pulp adventure to psychedelic 70s weird fantasy to bog-standard dnd tropes, to 3.5 animu bullshit, all in one mess of a place.
Over time, though, as the setting got as fleshed out as the system, the developers got just as lazy with the former as they were with the latter. Then, of course, you have veterans leaving and less-skilled newbies coming in, and they have different ideas about what works and what doesn't, which clash horribly, so what was previously a delicately-balanced mess just sort of collapses under its own weight and fails to be interesting - and that's without even bringing in the ever-skubtastic 'personal politics' of the various designers.
