The Inquisition. Wasting perfectly good gene seed and equipment instead of just sending them into some God Emperor forsaken backwater part of the Imperium on some kind of Crusade against significantly stronger forces. Also now that Papa Smurf returned he might start asking questions, at which point the Inquisitors who...nudged the events into their place could find themselvs on the wrong end of a boltgun, courtesy of those of their colleagues who wants some loose ends tied.

ironically if you read the book, the inquisitor throws a literal tantrum at a space marine apothecary sergeant when his order to not go on a rescue mission was ignored by the sergeant.
also the commander that needed to be rescue later became chapter master lol
