Had a session where the party were walking along the bottom of a great lake in the underdark. I took a lot of inspiration from the deep sea mermaid threads on here and the ocean floor section of SOMA.
The air in their diving suits was provided by a magical phonograph like a lot of magical items in that game. I played tracks made from that album softly in the background that would fade out into 30 seconds of silence before playing an alarm. If the players didn't notice the lack of music and the alarm played they'd suffer heavy damage and status effects, madness points etc.
They could play a new runic wax cylinder and start a new track, but they only had a limited number of them.
It took the death of the barbarian for them to really start paying attention to the music. Made for a great tense atmosphere.
