I actually do use index cards for all my characters. Sometimes I use 2 to 3 if it's a spellcaster. I write really tiny though so it works out fine. I also copy monsters from the monster manual onto index cards so that the characters don't know what they are fighting by glancing at the manual while i have it open to six different pages.

Not even close to the best anon. It's jumbled and confusing. It borrows a lot from the WoD character sheet without understanding the whys and hows of it. Why do Pleasure and Arousal share a dot pool? Why is there only one transformation line? This whole thing is a mess really.
