Well, that sounds brutal and a tad unfair on elite armies. If it's the same for vehicles I would think we'd see a LOT less deathstars.

Sounds pretty reasonable. Maybe there should be some saves based on toughness rather than instant death. Guardsmen getting squashed by the ceiling falling on them is fine. Terminators maybe not. Maybe they could lose a wound or die if they've already lost a wound.

Terminators could just be temporarily stuck in the rubble like Angron in the Crusade. Not really dead but incapacitated for the battle.
