What makes the realm a bad place and the dragon blooded the bad guys?

If you're a Dragon-blooded born in the Realm, you have it great. If you're a Dragon-blooded born outside of the Realm, you get to choose between the coin and the razor because they don't want you to sully their breeding plans.
If you're a mortal born to a Dragon-blooded family, you get to be a failure your entire life for not Exalting. If you're a mortal born to the rest of Realm society, well, it's better than being born a mortal in most of Creation but you're going to have to bow and scrape to your Dragon-blooded overlords your entire life at a absolute bare minimum.
If you're born outside of the Realm, your best hope is that you're not in their sphere of influence. Sure, there must be some vassal states that are treated alright, but for the most part, the Dragon-blooded of the Realm have huge appetites that will be satisfied regardless of the consequences to the peoples, the societies, and even the landscapes that those appetites are going to be satisfied upon. In other words, if you're in their sphere of influence, you're probably going to be brutalized until you become part of a resource extractor designed to suck out everything that's good about your homeland so that some Dragon-blooded can snort magic cocaine off of your/your countryman/your countrywoman's back.

They're decadent and self-righteous, but while they don't do as much good for all Creation as was possible with the Early First Age Solars, neither do they do anything close to as horrific as the Late First Age Solars.
In the long run, unless a cure for the Great Curse is found, Solar rule is a terrible idea - while it's probable that self-aware Solars could keep themselves in check if they knew about the Great Curse, the risk is that they'd change their minds, and by that point, there's nothing you can do about it.
Lunars are pretty clearly demonstrated to be shitty, shitty, leaders who should never be allowed within a thousand miles of a social engineering project.
Sidereals have their hands full being the Invisible Hand, and if they were overt rulers would probably screw things up even without the Great Curse.
Infernals... well, they range from Chairman Yang to Hank Scorpio, and it's really unlikely they could persuade the Yozi to forgive mortals.

Alternatively, you can just not go to the Realm and do whatever you want in the Threshold. Just march into any large city, yell "Dragon-Blooded for hire!" and watch the rich and the mighty compete to make an offer you'll accept.

They're the people in power. They like being in power, they do what they need to in order to remain in power, they use that power to satisfy their own desires. This isn't really any different from the way regular mortals act, or how First Age Solars acted. Still, Dragon-Blooded of the Realm are the people who are currently in a position to lord it over the rest of the world, so they are easy antagonists for most of the Creation.
