If they're low in number, but widespread they need some way of actually organizing and traveling. The mutations more or less preclude normal means of travel. They fight to earn the right to be CSM but have few actual CSM in their ranks.

They're the remnants of a SM fleet. Most, if not all, of the Astartes are dead and the warband is descended from the surviving Chapter serfs who crewed the vessels. That allows them to easily traverse the stars, but they can't really use a lot of the weaponry on the ships as it's all designed for Space Marines.

Their leader is a necromancer who enhances their number with zombies. The human cultists are more like Ghouls, if you're familiar with WHFB ghouls. Not true undead, but stricken and diseased men with a hunger for flesh, swift and debased. The greatest among these rise to become Space Marines, who become their few ranged elites.

It doesn't really specify they're few in number, just that there are few marines and many cultists. They could be numerous or few.

They could be, if we take your idea, whats left of a decimated Space Marine chapter, and the Sorcerer is a former Librarian who turned to the lord of death and necromancy to save his chapter from dissolution - raising up the dead, and preserving the living in stagnation for all eternity.
