Madness is discarded either way, it only passes through exile on the way to the grave, and you have the option to cast it from exile while it's there, but otherwise it continues to the grave and becomes eligible for Shadow of the Grave.
If you cast it for the Madness though, it is not eligible for Shadow even after you finish casting it and it goes to the grave, because you never discarded it, you cast it.

It doesn't say they have to have been discarded from your hand to the grave, only that they have to have been discarded at some point in that turn to somewhere, and that they have to have eventually ended up in your graveyard.

You're really having trouble with this. From the Comprehensive Rules:

Whether or not the physical card was discarded into exile, the game does not recognize the card in your graveyard as one that was discarded.

Under the current rules, Shadow of the Grave will never return any discarded card that has Madness. They may change this with the Amonkhet rules update, but they may not.

the context of being a flashback really makes it a lot different IMO. you can't evaluate it like you're playing it from your hand - the value is totally different in the context of flashing it back after doing your ramp thing. being an Aftermath card is not as good as drawing an extra card but it's way closer than you think.

I'm not saying it's super powerful, or even necessarily playable in Standard, but the rate is fair

Lost Legacy does see some minor sideboard play but the clause on not being able to hit artifacts is a problem because it couldn't hit not just vehicles but the gearhulks as well.

Don't evaluate this card like a comparison between cranial extraction effects only but evaluate it more like the card choice when one puts it into lists like the choice between Duress and Despise. There are some matchups where Duress is dead or better and some matchups where Despise is dead or better.

Modern Elves doesn't play Craterhoof due to the lack of huge mana of Gaea's Cradle and Birchlore Rangers/Wirewood shenanigans and its basic land count is like maybe 5 or 6 at the most. It's much easier for them to play Ezuri considering he's much easier to Chord for and hit with CoCo plus the mana investment to cast and activate him is exactly the same.

I'm guesing the original version made wurm tokens already blocking fliers - which means they didn't NEED reach, they just Already Blocked Them, like Flash Foliage here, but then nobody attacked with fliers at all so just simplified it
And then you still get wurms that can block landbound things regardless. Might be some thing from Creative where they can see aerial things easier than landbound or some shit to try to distance them slightly from their incredibly obvious inspiration.
Alternately, fliers can see that there are giant fucking wurms what will eat them and thus refuse to attack, while those on the ground cannot and thus walk right into the 5/5 wurm token

I miss cheap decks that could have a decent winning rate like Heroic in Theros.
Those were good times were lande in Top 10 or even Top 5 playing MonoWhite Heroic and UW Heroic.

It's interesting to me how much the current shape of Standard affects our (or maybe just my) evaluation of this set

Really feels like so much of the value in this set is about being more control-y, or at least more oriented towards the long game - most obviously, with mechanics like Aftermath and Embalm, and with the brick counters. Which is kind of a weird fit with a Standard format that's enormously quick right now. Be interesting to see how it plays out.
