I like hydras. They're a big carnivorous lizard plus a bonus scary thing (extra heads!), but unlike dragons they haven't accumulated a bunch of other baggage.

I also like shadowy monsters with horse's skulls. I think I stole that from an image that used to get reposted on /tg/. But horse skulls are awesomely freaky.

I prefer devils, the tempters and the deal makers. Nowadays players bitch and moan but if you have the right group then the look of fear/wonder they get when they realize that the innocuous task they performed in exchange for all that sweet gear accidentally set off a huge cluster fuck is great. Then they get to set off and try to fix it. Fun stuff, easy to write.

it makes for a large intimidating fight that is more drawn out than other fights due to higher hp but less damage, it is a good excuse to add special attacks that you might find fun or interesting, like the stiched in beholder eye opens hazily and fires a lazer or maybe even got a zombiefied dragons head, allowing for a necrotic dragonbreath.
then again, i love using undead, they come in massive variety, does not create any moral dilemmas and makes the group feel like heroes. and you know that past the shambling horde is a powerful necromancer or a lich. it's clichy, but the best kind of clichy

Fucking Aboleths. They're the perfect Greater Scope Villain with their patient scheming spanning millenia, they're brilliant when it comes to creating intrigue by manipulating NPCs, they can have a wide variety of enslaved minions and forcing the party to gear up and march into a fucking ocean to face this monstrous threat makes for a truly epic climactic confrontation.

One of my favourite things to do as a DM is to have an Aboleth's plans interfere with the plans of another antagonist group, like an enemy kingdom or a cult of Dracolich-worshipers. It makes for very interesting drama while the players decide how best to take advantage of the situation.

Plus the 5e Aboleth's design makes my willy bigger.

In my opinion, you can't have a sword and sorcery story with dinosaurs. They combine the mythical and the historical, allowing the players to imagine not just fantasies but epic counterfactual adventures. They open the campaign to kaiju battles, weird dinosaur cults, and time wizard shenanigans. Dinosaurs come in a huge variety of shapes and sizes to pose various challenges: from outwitting a Utah-raptor to surviving a stampede of triceratops. Let's not forget the epic victory won by defeating a tyrannosaurus. Especially in a low-magic campaign, a gritty hand to tooth battle with a dinosaur cannot be beat!

Well if that's what you're going for then dire rats lack a little subtlety. They're basically the bridge between normal hit'em till they're dead monsters and something different.

For something REALLY creepy, insidious, and potentially scary I'd suggest looking at cranium rats and moon rats.
Cranium rats are far more intelligent than normal rats and act as spies for illithids and even as the eyes and ears of their god directly.
Moon rats appear to be normal rats but as the moon becomes more full they become smarter and stronger till at the full moon they rival people. Furthermore, they remember previous moons and can act on long-term plans a month at a time, so subtle that the connection likely won't be realised before it's too late.

Liches are my favorite. They're like a better vampire. They (most of the time) dont require fuel, they're immortal, they dont have any stupid weaknesses and they are freaking magic. "Oh, but what if you want to have 'the sex'?" Just magic up a solution of use a glamore. Boom! Sex having lich!

Also becoming a lich is totally on the individual, theres no back ally attack that turns you into one like a vamp, ghost or werewolf. You can't blame anyone but yourself.

Underwater types
Civilized ones who can be reasoned with
Burrowing underground types
Winged ones
Types that are half dragon instead of half snake for higher levels
Even ones whose lower part is that of a centipede instead of a snake
I even have a lamia techpriest in one of my settings and its oddly fitting

Seriously, you can't go wrong with lamias, other monsters that I love include harpies(but admittedly more of a magical realm than anything), skeletons, ghosts and robots (humanoid kind with guns and weapons), but none of them come close to the versatility that lamias can bring

I'm trying to remember the name of this one type of monster I've once seen described on here. It hides in a pit or a sewer in a small town, mind controlling them into killing each other or feeding him their loved ones or something. I think it had the ability to use some sort of sound or music to cloud your mind so that you wouldn't know it was there even if you were looking right at it? So even though this dude just showed up five minutes ago you have years of memories of him, and it's almost impossible to distinguish him from the other people you know.

And when you recall the memories of the parasite there are other new characters, who then become real and add their own memories, which is how the parasites reproduce.

Within a few hours you've got hundreds of people running around and you remember all of them like you've known them your whole life, and even if you know there are parasites about you have no way of figuring out who's a parasite and who's an actual friend.

Even if you figure out, can you handle the emotional trauma of murdering a dozen of your best friends? The memories don't go away when they die.

The end-game is the parasites' reproduction exponentially accelerates until the planet becomes uninhabitable for the billions of people appearing every day.

Goblins.

It is both a monster and a descriptor. Goblins use Goblin Metal, and ride Goblin Rats, or Goblin Spiders, from Goblin Castles lorded by Goblin Kings and Queens, who hoard Goblin treasure which can be spent in Goblin Markets. Goblin is as much a curse as a creature.

Best part, a goblin can be a Hobbit goblin, a Labyrinth goblin, a Gremlin Goblin, a Warcraft Goblin, and a Warhammer Goblin. They're greatly varied and all can work in the same world.

They can be silly, they can be sinister, they can be stupid. They will always be effective.

they can be anything you would like. Mindless monsters that could probably take on an entire town's guards if needed
Ones whose very breath could freeze their victims' blood in their veins
Maybe they quietly sneak into a family's home, kill their baby and take it's place - growing up human and one night killing and feeding on the entire family
They could be completely indistinguishable from a normal human, they spend several lifetimes in human cities feeding on the slumbering citizens at the night, not enough to kill but maybe they'd need to spend a day or two in bed with some unknown illness
Elder vampires so ancient and powerful that they act as a sort of "leader" for all vampires of a particular area, that get irritated by even the slightest noise and even the most powerful of other vampires fear even discussing him

there's so much stuff you can do with them, they'll always be my favorite

I love monsters that are like mundane plants and animals but warped and corrupted into something horrifying and new. It makes it easier to describe them to the players, not by saying "It's a weird jackal-thing" but by watching how jackals move and describing this quadrupedal, shivering beast with elongated hind legs, limbs emaciated but a bloated torso fit to burst, snarling and encircling the character.

Mechanically I like any monster that escalates in threat the longer the fight goes on. My last two boss fights I've run were (1) a giant crocodile monster that got an additional turn in the Initiative order when it hit 80/60/40/20% maximum hit points, and (2) a huge earth elemental that split off an additional smaller one at 80/60% max HP, and THOSE split off two even smaller earth elementals at 50% HP, and when the main one was reduced to 50% HP every elemental got a large bonus to attack and damage and a small penalty to AC akin to Barbarian Rage in 3E (which I'm not playing).

I don't run random encounters, I don't waste players' time with four goblins in a 10x10 room. I run every fight like it's a mid-boss or boss, throwing badass creatures with nasty specials and hordes of minions at the party. Monsters should be terrifying and a threat to their lives every time they encounter them.

The weird part is that it's easy for me to identify my favorite monster sourcebook— the Mutants & Machines book for the d20 Gamma World— but I can't pick out a favorite creature from it. I just love the way so many of the entries in it are the bizarre products of good intentions gone awry: the ooze that was originally a nanotechnological liposuction device, the intelligent vending machines that have been forced to learn how to dispense medicines and other practical compounds instead of soda, the oil-spill-cleaning otters that get high off of gasoline, or the buildings which uprooted themselves in search of replacement parts.

I guess my actual favorite would be the Xaxavraznazak from "Fire on the Velvet Horizon," but I like it too much to use it as an actual antagonist.

I think the illithid are probably the best D&D monsters. They're exceptionally well-developed and have a lot of character; I think they're closest D&D gets to a genuinely new yet iconic monster archetype, even if the popular depiction of Cthulhu also has the squid head thing.

The drakainia is uncomfortably close to my own fetishes, and I could never put one in a campaign, but I do like them a lot. Like the Xaxavraznazak, I'd never have the heart to use one as an antagonist, but I think a drakainia might make a good, if silly and prohibitively fetishy, ally.
