I actually stand by the inclusion of this girdle in games (specifically, the version that masquerades as a belt of giant strength or some other magic item), by virtue of the fact that it works well as Babby's First Cursed Item. That is to say, it works best when introduced to players who are relatively new to the game and the concept of cursed items.

Since there is almost never a mechanical impact in D&D or Pathfinder to being male or female (barring some niche prestige classes with gender requirements, like the Battle Maiden or Eunuch Warlock), a character who puts on the Girdle without thinking won't be gimped or penalized in a mechanical way. HOWEVER, since it's probable that their character's particular choice of gender was something they want, and they don't want to have to play as the opposite of that - and, just as likely, would like that belt slot open for a better magic item - it's still something that they're going to realize was a mistake and want to fix.

It teaches players to watch out for cursed items, without mechanically punishing them when they'd never encountered them before. Everyone has a good laugh, they get the curse removed at the next town, pawn off the belt to some unsuspecting merchant, and continue on their merry way, wiser for the experience.

Fortunately, I've had the privilege of playing with socially competent people, so I haven't experienced it myself. But you have to remember, when degenerate people are in a group it's a lot easier for them to feel like their actions are appropriate.

I'm sure a GM has figuratively shoved their fetish down their group's throat a least once, even through the resistance of the entire group. There's a reason the Magical Realm comic got made in the first place, you know. I think it's a symptom of lonely, maladjusted people that either haven't learned to control their impulses or don't even realize what they're doing is inappropriate.

You are quite mistaken, except insofar as Clerics are more likely to have access to the magic needed to remove it. But Remove Curse is a mere 3rd level spell in 3e and 5e (dunno about other editions, but I assume it is similarly low-level), so any 5th level or higher Cleric can cast it, and in D&D terms 5th level isn't all that high.
