Noir plotting is basically lose-lose. Your protagonists (the players) find themselves in a corrupt social, political, legal, or whatever system where they're either victims or forced by the system to victimize others. There are no "correct" decisions, no way to "win", etc. The protagonist(s) get out of the situation by literally getting out albeit at some great personal cost.
So, where to place your medieval noir setting?
Constantinople in the Byzantine Empire.
It's urban, rich, corrupt, and bureaucratic.
