Only if the accuracy or the damage is near zero or replace an existing weapon and I get to make up hilarious bullshit about how the character mishandling resulted in friendly fire.

Yes, since I run pathfinder and I'd just re-fluff the man-cannon as the double hackbut to not have the set-up tripod but a longer re-load time and splash or spread depending on ammo

If they're strong enough, they can lift it and handle the recoil.
If they want to use it as a regular weapon, then they have to deal with reloading it and the innacuracy at range and so forth.

Does it fit in the system currently being ran? If so then yes. If something can be handled by the RAW I will always let them present their case.
The real problems usually come from the requesting player not agreeing with the rules. Then they have to make a case to me to consider changes.
