The immaterium is everywhere, but the fucked-up chaos warp is a result of actions by Necrons, Eldar, and humans within the Milky Way galaxy. Other galaxies will have their warp in different conditions. Probably a lot of them empty and dead due to Tyranids.

There's some old rogue trader fluff about the Inquisition sending a probe outside the galaxy, can't remember what book exactly. It came back thousands of years later or some shit and it was Ork communication everywhere. A shitload of Orks everywhere. Maybe the Milky Way is just a small anomaly in the Ork Universe.
