how do you make an interesting combat-heavy 5e campaign that isn't just 'ok clear this room and then that room and then that room' without just disregarding the entire rulebook to do truly cool things.

Have character to the fights
Hell, even gimmicks can be good for fights. I did that several times. My favorite was a goblin with a whip that only tripped the PCs while the goblin's dogs did the damage and managed to escape into a deeper room of the cave twice. PCs enjoyed killing that guy when they cornered him.

I'm sorry to say that extremely combat-heavy games is not exactly what 5e does best.
Combat in 5e is done best if each encounter has a defined purpose and it's not there just to fill out time and roll dice because it's "expected". By making combat more a part of the objective rather then just "some monsters, and then you fight" and focusing on things like terrain and allowing PC's to be creative with stuff you can spice it up.
But just pure dungeon crawling combat WILL get boring with 5e's relatively rules-light system.
I suppose you could use the Battlesystem rules Unearthed Arcana they released for mass battles, but that might end up having the same problem.
Environment
Put in things in the environment for the sides to use to their advantage.
