Anyway, Riddle of Steel and its derivatives like Song of Swords are pretty deadly.

Are you asking for a melee system or a modern combat system? For the first one, Song of Swords. RuneQuest 6 is also great. GURPS can also be good depending on the rules used from Low Tech and Martial Arts.

As for modern combat, I have way less experience, but I've been told that GURPS is actually really good at modern tactical gunporn combat.

No seriously once players have tactics that can mitigate the death for players through actions the game is no longer 'deadly' to players. The rocket tag that is going on would have to have some control under the players meaning they want the first volley and just sorta hope they don't die on return of fire. Lots of tactics to set up and if they're doing it 'right' they shouldn't have any problems so the deadly bit is dropped. Now as a GM you should upset plans but under a deadly system if your NPCs get the first rocket tag volley you're almost at a TPK and none of their tactics mattered because of general GM giving them the fear of god and honestly interrupting a boring streak of 'your plans work'.

40k Fantasy flight games sell themselves as deadly but at it's core the system makes players really survivable and has a lot of a pistol shot directly to your head probably won't kill you.

You are a retard for thinking like this. I've been running RQ6 with no character deaths and no punches withheld because of good tactics from the players.

And it's not just roleplay, once you are in combat there is a lot of important decisions.

There's actually quite a few. Thing is most of them are fairly older games or derived from such because today's gamers are the generation of participation trophies, No Child Left Behind, and "everyone's a winner."
