If they're on the mound they're called a pitcher, but I digress. He can throw the ball any way he wants to, yeah, but it'll either be a strike, ball, or foul.
So for a pitch to be a strike it can either be in the strike zone (pic related) when it passes by the batter, outside the strike zone but swung on and missed, or inside the strike zone and swung on and missed. A ball thrown for a strike is still in play if the catcher misses it, so sometimes a player that strikes out on a passed ball will run to 1st base and it is not counted as an out if he makes it there before the ball.

A foul counts as a strike until a batter has 2 strikes, then if a batter keeps hitting fouls past that point the batter does nothing but rack up the opposing pitchers pitch count. A foul is a hit ball that goes into foul territory on the left and right sides of the field (the two lines going out from home plate to the big yellow towers at the other end of the field) or behind home plate. A hit ground ball can also be foul if no player has touched it and it rolls into foul territory before leaving the infield. Each field has playable areas foul territory, so if a fly foul ball goes there and a player catches it, they're out. They also can strike out on whats called a foul tip, which is when the bat has hit the ball, but the catcher still caught it.
A ball is simply any pitch that is outside of the strike zone that isn't swung on. So a pitcher throwing over the batter would do nothing but walk him after doing that four times.
Most of these calls are done by the home plate umpire, the guy in black behind the batter and catcher. However sometimes a batter will have started to swing on what will be a ball, and they try to stop their bat before it passes home plate. Whether or not they were successful in doing so (and by extension whether that pitch is a ball or strike) is decided by the first or third base umpire, depending on whether the batter is left or right handed.
