Two giant issues, battles with weapons to the death would be incredibly short and ugly fights, additionally you'd have little emotional investment behind the matches as there's really no way to build feuds when after your first loss you die

Nope, Gladiatorial fights actually weren't that lethal. Obviously a lot did die but mostly criminals who were forced into it, actual professional fighters cost a lot to train and keep in the game (much like pro boxers) so they didn't often fight to the death. It was usually until first blood or the fight became unwinnable for one of the competitors.
