There are two teams. That adds up to 3 minutes after they use all subs. Then take one minute, on average, for actual injury stoppages and another as a sum-total of other stoppages (goals, fouls, etc) and there's your five minutes.

Even if stoppage time has expired, referees don't blow the halftime or final whistle until the last attack ends (ball enters midfield third). The one thing refs sit back and let play out, especially if a goal on the final attack can swing a match's outcome (remember: sports are entertainment).
