I once broke my arm playing soccer in first grade. I tripped on the ball and landed on my arm with all of my body weight crashing down. Imagine taking about the halfway point of your forearm and bending it at a very sharp 90 degree angle upwards. The bone didn't pierce the skin, but you could see it almost penetrating and joilting up from underneath my skin. Wish I had the X rays with me to show you lads.
Anyways, I bring this up because I was very fortunate in the ordeal. I didn't feel a thing. It didn't hurt one bit. I was just playing soccer one moment, and all of a sudden blacked out. When I woke up a few seconds later, kids around me were screaming and freaking out, and I was genuinely confused. The body going into shock can be an incredible thing. Makes me wonder if athletes like in OP's image too were spared or pain.
Anyone else who has broken bones, did you feel pain? If so, how bad was it?
