What would happen to you if you were lost in the wilderness and had a dislocated shoulder? What would the long term affects be?

Death. If the dislocation didn't just result in massive internal bleeding then having a viciously painful arm that cannot be used would mean you couldn't get food, walk far or make shelter.

You must attempt to fix it straight away. It can get a lot worse, if you leave it. You can also make it a lot worse by trying to reset it. Sometimes the damage will be too great that it doesn't matter what you do, you'll just die eventually.
At worst you will die due to extra damage (but no the dislocation itself).
At best you will reset your shoulder with minimal extra damage, you have to allow it to heal after the reset so making a sling is necessary too. It would be extremely hard to reset your shoulder in the wilderness alone. If you had someone with you it'd be a lot easier.

You can fix a dislocated shoulder by yourself. You just grab your arm by the wrist and pull downwards until the joint realigns. It's a bit hard to do if you are not strong, but anyone who does things that dislocate shoulders is probably already in shape.
