Random fasciculations from time to time are meaningless -- persistent ones in the same place probably need to get checked out, can be early signs of several neurological issues.

As far as I remember, they basically don't happen with active usage of the muscle. The nerves can't misfire if they're constantly set and reset.

Low potassium or other electrolytes (but mostly potassium) can cause misfiring of nerve and muscle impulses. This is the most common root cause of most muscle twitching. Its mostly seen in the eyelids but can effect any muscle group in the body. Same thing that causes muscle cramping.
