Simply put there are proteins that cells produce/inhibit in order to establish axes around the body
It's a very clever way of cell communicating with each other and telling each other where they are localized in proximity to the axes, you left arm knows it's on the left side and produces a left hand with the proper rotation and positioning of fingers
although in most cases hands are produces in "bulks" and then cut into strips to make fingers
Now as we all hopefully know here on /sci/ all genes code for proteins and proteins only
So what has happened is that the cells in that person's toe had a mutation for a protein used in communication and hasn't established an appropriate axes and has told the wrong cells to specialize into making a toe
