There's an entire branch of philosophy called "philosophy of pain." It gets very abstruse.
In general usage, we assume that pain is a subset of stimulus in general, and stimulus is supposed to correspond to some kind of physical outcome.
So if you have the sensation of your hand being crushed, you would typically expect your hand to be crushed. If it turns out that the pain was just a mental simulation, then you felt A hand being crushed, but it's not actually clear that it was YOUR hand, as opposed to a mental map using your physical hand as an approximation.
But the ultimate point it, it's still going to hurt a lot. With willpower, though, you can learn to ignore pain.
