Start by taking small steps now.
Don't focus on doing everything all at once or you'll psych yourself out.

the only way to truly kill procrastination is a ~100% eradication of the bad habits that fuel it. This I figure takes months to do, a summer break perhaps. Physical fitness is integral to this, establishing healthy habits and meditation to search for contentment.

Does anyone have Cal Newports book pdf link? I haven't been able to find one, and no library in my area has a copy (I don't want to interlibrary loan it because last time I did that with a popular-material/non-scientific book the librarian gave me dirty looks.

I find that i dont procrastinate because i dont really think about it. Yup thats my secret.
I either do the thing right as soon as i remember it or say, in the next five minutes. And come that five minutes i then do it.
Ignorance is strength i guess...
Actually i think theres more to this secret, its that i always do things exactly when i intend to do them, and if i really fuck up and put it off untill the night comes, i just shrug and say "i guess i needed a little more time off than i thought". I dont know if this helps. But its basically compassion for oneself and ones predicament. But the important thing is that you dont lay down the behaviour pattern of putting off 5 minutes, then 10, then 15. Just get really used to doing it.

Exercise really washes out the brain of emotions which might drag on past their use. These emotions might be a cause of procrastination.
