I only had to apply to 3 places before I got an offer.

Perhaps there is something wrong with your application and cover letter. Get it checked by a pro or a friend.

My advisor said he liked my cover letter and my uncle (has a JD and PhD) said my CV was solid.

Letters of rec were great too.

I think the biggest red flags on my app are mediocre GPA (3.56), GRE quant. (76th percentile),
and applying close to the deadlines (like, REALLY close)

Well, my SO had applied to 10, interviewed with 2 or 3 (I think 2), and got 2 acceptance letters.

That was after a second round of applications. My SO finished undergrad in 3 years, so I think most of the programs didn't want to accept someone that young? Or maybe my SO just wasn't lucky.

I don't know, but, that is how these things work. Just go with the flow and don't stress out too much.

This would be your biggest problem, but I don't know if they would count that against you or not. I think some programs do. It probably depends on the school, and I think it looks lazy/sloppy.

I worked for a head of a biology department. He said that they don't put too much weight on GPA and standardized testing. Basically, as long as you were a bit above the min. req. you should be pretty good.

Mostly, as long as you have stellar references and an application letter, you should be good. I think they are looking for proof (stories/character/etc.) that show you will be someone that they want to work with in the future.

Not necessarily. GPA is one of the less important factors in grad school admissions. Typically, GPA is used to screen applicants; often, there's an index that combines GPA and GRE to measure applicants' scores.

But, once applicants pass the cut-off, GPA and GRE are no longer factors. Research experience is the most important factor, followed by letters of recommendation, followed by statement of purpose and/or personal statement (varies by school, but why the applicant wants to go that program and why a PhD).
