Does true selflessness actually exist, or are all humans just selfish pricks who almost always go the path of least pain?
What is true selflessness? Taking only what you need?
some might think they're selfless, but they're just delusional

Yes it exists. Prooftext: some people really do throw themselves on grenades to save others.
How can selflessness really exist?
People will only do what makes them the happiest. Even if you jump in front of a train to save a loved one, you're doing that because you'd be happier dead (Which would be zero happiness) than you would be alive without that person (Which would be negative happiness.)
If someone says "I couldn't bear living without you" and you feel the same way, and they insist they die instead of you, wouldn't that be selfish, since they know you'd be unhappy living without them?
Of course, it might be wrong to assume that it would be negative happiness for someone's entire life without their loved one, but humans aren't good at thinking about things long-term.
Yes, people who have experienced ego-death are selfless, or were at that point in time at least.

What you mean with "true selflessness?"
Like, If I make a gift knowing that I get nothing in return? Yeah, it does.
Now, if you mean a completely selfless human that always prioritize others, then yes, but it's because they also gain happiness from feeling helpful and good, not precisely "nothing in return" in this case, but the trade is completely altruistic and selfless.
