There is no such thing as a Diagnostic Medicine. Doctors specialize in specific ailments, not all of them. Master of all, expert in none.
Also, the speed and amount of tests they do on patients is unrealistic. Insurance companies would not sign off on 90% of the tests he requests.

A combination of fraud, Cuddy deliberately bargaining with the insurance companies to get there way, the hospital having to fight tooth and nail over each charge, and the hospital ending up covering a lot of it. Mostly the first one though.
