MPC (Media Player Classic) makes excellent preview images (Menu: File > Save Thumbnails ...) that can be customized with a simple interface. The whole process takes a few second per video. I've uploaded a sample image using 3x12 with default settings for reference.

It seems like there is a middle ground where accidents happened at such low frequency that it can be dealt with using photoshop without too much hassle.
